Standard Notes are not Appearing on the Account

If the standard note from page 5 of the smart code does not appear on the account check the below:

The standard note will not appear if a user keys in manual notes unless there is also an "A" in the field duplicate notes (Y,P,A)

NOTE: An alternative resolution would be to have the note add from the action line instead of the page 5 standard note field.
